This was my favorite utility type app but suddenly its losing data. When I first installed it a couple months ago, it updated on iTunes & I lost our Dec- Feb sicknesses with that update & i had to start over. Ive been using it DAILY for the past 8 weeks for a chronic condition that has flared with increasing frequency, making me have to update notes / symptoms etc. from 1-3x a day. I entered todays note, symptoms, & pain scale at 4:15 & updated twice more & each time it lost the updates. Actually, each time I updated, I reentered the lost info with the update & each time it reverted to the first entered info. This affected the notes, pain scale, symtoms, & medication fields. Great concept & very useful but only if it works. And since this is regarding peoples health, it needs to be virtually flawless! It cannot have lost data! Correct the bugs & it will rate 5 .

I wrote the creators some suggestions, and, whatdya know, they appeared in the current update! Ive rarely found a developer so responsive! This is a wonderful app. It allows tracking individual symptoms, if you are running a fever, seeing a doctor, and, now, the medication(s) you took, the dosage, and the time you took it. I have no reservations in recommending this app wholeheartedly! And this is from someone who has more than a passing acquaintance with the medical community.

Dont buy this app! You cant type in landscape which is EXTREMELY IMPORTANT when entering medical info in a doctors office! Ever since the iOS 4.0 update it deletes my entered data. This is not a game app and I need perfection and accuracy when documenting. I have a mentally ill daughter who has many needs, this app covers a minimal amount presets to help you quickly document. You will be entering a great majority of info if you have a procedure, sick with more than a common cold! My Period Tracker app is WAY BETTER than this and it allows you to backup data to your email. I AM SO ANGRY, this app deleted my daughters vitals and others important info! I really miss a good old fashioned paper date planner like the Go Mom™ planner that is no longer available. I rely on this app, its a shame I lost data from 7/12 TWICE! Waste of money!

This is a wonderful app for tracking acute illness. If you have children and need to track fevers and medicine given, it works very well. A good way to track antibiotics taken and otherwise manage short term illnesses. Unfortunately it does have a habit of losing data so it needs to be reentered. For chronic illness, there are better choices. This app only allows one pain number per day and youll spend all day typing out what are areas of pain. Plus the loss of data is more frustrating when youre trying to use the app over a long period of time.

I find almost everything about this program to be perfect, except one minor (cough cough) glitch--it randomly drops some of the data inputs. I make an input and push save; I then confirm it is on the calendar-good. Then a week later its gone! Sorry, but I need absolute reliability with this program or it is useless. Right now, it is useless.

Much needed app with a lot of potential. For someone with a chronic illness with regular flare-ups, this is very helpful for tracking dates and symptoms for my doctors. Would like to see a copy function for symptoms so that if Im ill for a few days and have the same symptoms, I dont have to keep entering them each day. Thanks!

Love this app! My childs doctor needed some info and I was able to send it via email. Love that I have info on each family member. I have never had the drop issues others mentioned and I use it weekly. You just have to remember to save it. My doctor liked the apps email feature so well shes telling other patients about it.

Worked wonderfully at first, to the point that I was able to rely on it to keep accurate records of my daily symptoms and medications, but after a couple weeks of usage it deletes records, specifically medication, times and dosages Im entering! Very frustrating that I am no longer able to use this PAID FOR app in the manner it is marketed... This issue needs to be addressed and corrected immediately when dealing with peoples health and safety the technology should be more reliable and up to date!

Exactly the type of app I was looking for. I would really like to see other types of scales, like the pain scale, but for overall well-being and/or a scale for depression and anxiety levels, and/or customizable scales. And would like to also see a feature to enter your weight daily, and also be able to choose how many times you experience a symptom throughout the day, such as how many times you go diarrhea.

I have yet to find another health app that easily lets you track medications and symptoms.

I have been using this app for years and it is extremely helpful. I have a child with tummy issues. I document everything and when we see the specialists they are always so pleased to have so much information to work with. I only wish the was a way to add pictures in to the note section.

I have a chronic illness with many appointments and hospital visits. Ive entered information, but there is no search function. What is the use of imputing data if you cant search for it?

This was the most perfect medical/illness diary app I have found in close to four years. I could easily create a list of my pills, as I took a painkiller as needed I could select it, it would add a time stamp and go on "todays" date, same with my symptoms. I could add any of my personal symptoms and select them from the list as they occurred and it would be added to "today" on the calendar. Ive used this for over three years to take to my weekly doctor appts and could easily scroll through the calendar and read off things in order. Or, when I took a painkiller, I would know how long to wait to see if when it kicked in it worked or I needed another. OR before I had this, sometimes I forgot I took a pill, would take another and realize later (through a haze) that I doubled up by accident. This was never optimized for the iphone 5 but it functioned. Now it doesnt work at all on iphone 6 plus. So far Ive bought two similar apps only to find out they dont do what this app does at all. Im so upset. This program is irreplaceable and Im wondering how easy it would be to learn to create apps myself to duplicate this lifesaver.

Needs a major update! Should be taken off the App Store until updated!

This was an awesome app. I have used it for a very long time. Now it doesnt work at all for iPhone 6. Please please fix this.

Do not bother wasting you money! Scam why dont they put in the description that they havent updated since 2010?!! I should have been more careful true, my fault but dont get fooled too.

Needs a major update!!! Sometimes what I enter and save disappears. Im sure if there are others like me who get so busy throughout their day that they forget to take their medications, an alarm or reminder would be icing to the cake of this app. I would also like to be able to sort by individual medication, kind of like track specific individual meds. And maybe correlate the pain (1-10) with medications taken and mot taken on particular days

Ive used this for almost two years and about 5 months ago it started acting up and freezing. This is the PAID version and the manufacturer WILL NOT RESPOND via their support email. Apple should remove this App. Its a crime to be selling it!!!!!!

It will not work. App is frozen no matter what you try. Do not waste your money. It has major bugs and needs updated. Shame on you for selling this. Apple needs to remove this app being sold. It does not work. I want my $4.99 back!!!! Not kidding.
